INFORMATION BACK GROUND AS PER ALLOPATHY

• I will concentrate on INFECTIOUS DIARRHOEAS ONLY – which will manifest as acute watery stools when the same is stained with VISIBLE BLOOD INSTOOLS the same is called DYSENTERY; if such episodes last more then 14 days they are called as PERSISTENT DIARRHOEAS.

 

• The chief causes are BACTERIA; VIRUSES, PARASITES.

• EPIPEMIOLOGY – Worldwide 2.5 million die each year secondary to diarrhoea mostly developing  countries.  It is estimater that 20-40 million diarrhoeal illnesses occur in children age less than 5 years in the united states alone (SOURCE PAEDIATRIC GASTRO ENTEROLOGY.

• It is an accepted fact it is extremely  difficult clinically to decide whether a given case of infantile diarrhoea is due to bacteria, virus, parasites, because climically they cause similar manifestations.  So there is no way clinically to decide a given case is of VIRALORIGIN OR NOT.

 

• In addition wide variations are present depending on seasonal and geographical differences.

• Finally children with compromised immune systems, secondary to MALNUTRITION, immune deficiency, and imuno suppressive medications are susceptible to a broder array of pathogens.  

 

• 25.4% are caused by ROTAVIRUS, besides NORWACK VIRUS calicivirus; Astrovirus, enteric Adeno Virus (Nonopportunistic) Opportunistic virus,   Adenovirus; cytomegalovirus; Epstein – Barr virus, Human Immuno deficiency virus etc.

DISCUSSION

The above discussions  are narrated to appraise readers to make sure that they are treating an intectious diarrhea  with to be written Ayurvedic formulae which are meant purely to treat epidemics so case selection is VERY important to make sure they are treating only infectious ones.

TREATMENT OF DIARRHOEAL DISEASES BY AYURVEDIC REMEDIES :
(From Ancient Ayurvedic Book “Charaka Samhita” “Chikista Sthanam” 

In order to treat Diarrhoeal diseases one has to understand its varieties as per Ayurvedic.  They are as under with symptoms and signs.

1.       “PAKWA VAATHATISARAM” – SAMPTOMS :]

          As there is no corres ponding English word all the readers have to accept above terminology.  There are loose motions with FROTH; mucous and “back Saw” cutting like pain in the anal region; frequent passage of flatus withhigh sound and motions quantity is small and patient has to exertabit before passage of stock.  There is dryness of mouth, body pains, here motions appear to be passing as a small balls.

2.       “PITTHAATHISARA” SYMPTOMS :

Here loose motions will be coloured yellow, leafygreen, blue ‘Krishna Varna Telugu’ means light blue, either of above colours, at times mixed up with blood also, foul smelling frequency is more.  Patient will feel thirsty, complains of burning sensasation in the body, sweatings, semiconcsiouness, burning around the anus with ulceration around the anus.

3.       “SHLEMAATHISARA” SYMPTOMS :

Here loose motions will be pasty; white coloured, with mucous which spreads like a thick thread; appears as if it is consisting of indigested food, ofensive odour; heavy quantities, with pain abdomen; frequency of passing motions is more and more with tenesmus, at times patient has to exert to evacuate fully; and at times patient will not be knowing that he is passing motions; there will erection of body hair; nausea is masked; no appetite will be there and no taste is felt in the mouth, there is no desire to take any type of food.

4.       “AGANTHU-SARA-JATHI” – ATHISARA SYMPTOMS :

There are two types described by the Author in Book one variety is caused by SORROW, GRIEF; and the second one is caused by “FEAR COMPLEX”, both of these have same symptoms as said in “VAATHATHISARA” above No.1 variety.

5.       “SANNIPATHATHISARA” SYMPTOMS : and causes :

The Author in his book described many reasons  for this.  To name taking fish mixed with milk or taking one after the other – Taking unclean water, High Temperature such as summer. 

Floods, psunamis, weakness, “Vaata; Shleshma, Pitha” together (Ayurvedic words no English equivalents available)  “Doshaas” so that above there varieties of symptoms will be seen as “MIXED” inthis type of “Athisara” = Diarrhoeas.  In this there will be very very loose, blood mixed, light blue, white, pig fat resemblance, with pain and one or without some times indigested food motions, at times digested, loose motions will be be there.

Patients who are having following symptoms; signs will be assumed as SERIOUS PATIENTS WHERE RECOVERY MAY NOT BE THERE :
The patients are said to be with “BAD PROGNOSIS” if they have loose motions.  Looking like caused blood; pure fat; curd, ghee; bone marrow like, milky, dark blue; different mixed colours; lot of mucus, pure yellow, chocklate coloured, having smell of a deadbody, frank pus, fishy, mixed foul smelling, and there will be countless number of houseflies  on surface of stools; stool will be heavy, or at times not at all; at times avery small quantity,   symptoms like fever, burning in anus, entire body, delusions; stupor; Hiccups breathlessness; ulceration  around the anus;, prolapsed anus; rectum, marked dehydrations and stool getting stopped all of a sudden without any sort of treatment.  If above symptoms are there in a given patient relatives should be informed that “recovery is not assured”.

In “sannipaathathisara” there will be mixed symptioms of “Vaatha, Sleshma, and Pithha” Telugu (no English equivalent available) – in symptoms which of the variety symptoms are dominating, treatment should be directed to that variety.

At the bedside; or in large epidemics it will be difficult to manage unless there are proper facilities like inpatient facility and supporting staff and availability of various medicines.

While  discussing Ayurvedic treatment writer will not bring ALLOPATHIC APPROACH in Ayurvedic treatment.

Can we combine both is a POLICY MATTER to be decided by experts in all fields of Medicine.  Remedies to be described will be only AYURVEDIC and nothing else.

METHOD OF PREPARATION :

• “VASTI” means well washed dried urinary bludders of dead animals like Goat, Cow, Bull, etc.  The nozzle  with a hole is called “NETHRA”.  This will be instroduced into the rectum.
• Now a days modern “Enema set” can be substituted.
• Take equal quantities of ingredients. No. 1,2,3,4 and grind them into a fine paste.
• Take wide mouthed stainless steel versel with lid and largest spoon to mix.
• Add GINGILLY OIL 1/4TH of the above volume (equal to) and ¾ volumes of cow ghee and add to this 04 times cow milk and slowly heat the contents on a stove run by steam and mix up regularly so that materials are not over fried.
• Boil to such an extent that the original volume of oil + cow ghee remain in the vessel.
• At this stage separate the oil + Ghee mixture by FILTERING THROUGH CLOTH OR FILTER SO that only oily portions remain in containers.
• After cooling use it AS RETENTION ENEMA.
• If oil ghee gets evacuated the same process may be repeated next day.
• This method is supposed to be the best form of treatment for “Pittathisaram”.
• GIVING HERBAL RETENTION ENEMA IS AN ACEPTED SURE MODE TO TREAT “DIARRHOEAS”

 

FOR RESISTENT “PITTATISARAM” “PICHHAAVASTI”

METHOD OF PREPARATION :

• After taking stems/anthers of fresh flowers of item No.1.  Wrap them with green grass to make them as a BUNDLE.
• ENGULF WET BLACK EARTH around that above bundle.
• Put over clean place and put fire with “DRIED DUNGPIECES”
• Keep it to such time that the “Earth” over it get  to “DRY STATE”
• Take it from “DUNG FIRE” allow it to cool.
• Put these cooked item no. 1 and item No. 3 in a grinder and grind it to make in to a fine paste.
• Put the mixture of above 02 items in a stanless steel vessel and add 640 gms of cow’s milk and again grind them adding equal quantity of GINGILLY OIL.  ALLOW IT TO COOL.
• Note the resultant product described  above should be a very very fine oily product easily administer able via “Nozzole of Enemaset”.
• It is always desirable that “VASTI” to be  administrated  to patients in left  lateral position.  In leg down over stomach exposing the ‘anus’ properly.
• Then administer the above “oil” + Ghee as retention enema; and after its automatic evacuation.
• Give oily massage to the body of patient and then give a pleasant bath followed by easily digestible food to the patient can take plenty of water to drink.

 

THE RETENTION ENEMA TYPE MEDICATION IS SAID TO GIVE BEST RESULTS EVEN IN VERY VERY RESISTENT CASES OF “DIARRHOEA”

It is said to improve general health also of the patient as the absorption of oily medicine is achieved from large intestine, etc.
This TYPE OF VASTI IS HIGHLY RECOMMEDED.

MODE OF USE OF ABOVE MEDICINE :

The above medicine has to be mixed with “MANDAMU” telugu = a sort of cungee made of unpolished rice prepared by taking -01 part of rice and 014 volumes to it of water and boil it till no more rice is visible (As described in ancient Ayurvedic Book “VASTUGU NADEEPIKA” page 584.  Immediately the patient has time with “PAYAMU” – Telugu details given below).

Dosage and duration : As directed by the physician.  In Telugu “PAYAMU” means TAKE 1 PART of unpolished rice add 014 parts of water boil it tillRICE PARTICLES ARE VISIBLE AND APPEARS AS PASTE LIKE.  Eat it well after consuming the medicine  as directed above : (As described in above book at page 581)

“RAKTAATHISARA – SYMPTOMS & SIGNS”
          When even diarrhoea is associated with frank blood in it is called as “Raktanthisara” (Bloodly Loose motions).  This is the result of improper treatment  of “rithhathisaram” but the original Author never accepts this as a seperate entity.

FORMULA TO HEAL ULCERS AROUND ANUS OR SORENESS :

• Alternatively make fine powder of above 05 ingredients add Goat Ghee / cow Ghee / apply after mixing the ingredients thoroughly.  Alternatively spray fine. Powder over ulceretc - apply

METHOD OF PREPARATION :

Decoction means “KASHAYAM” in Telugu.  There are 05 types of described below.  Readers should be familiar with this viz;

• “SWARASAM” Telugu means PUREJUICE OBTAINED without adding water.
• “KALKAM” Telugu means if herb is fresh  grind it and making a  paste, if dry herb add water small quantity and making it a semisolid paste like preparation.
• “KWATHAM” (Telugu means a decoction) means taking o1 part of herb and making into finest pieces and then adding 016 parts of pure water and boiling it till 1/8th volume remains.  Then it should be cooled and preserved.
• “HIMAM” (Telugu) mean a variety of decoction made as under 01 part herb (s) grind it add 6 times pure water and keep it stored in a vessel for 24 hrs. in a cool place then filter it through a clean cotton cloth No boiling.
• “FAANTOM” Telugu means one part of herbs and putting them in HOTTEST water in a vessel and allow it to cool by itself, then filtering it through cotton cloth.  This liquid is called ‘faantom’

ACTUAL PREPARATION OF MEDICINE :

• Take item No. 1, 016 kgs and grind them collect pure juice by filtering through a cloth of cotton, add equal quantity of cow’s milk to it in a clean stainless steel vessel.
• To this mixture add 02 kgs 560 gms of “COW’S”  GHEE.
• Add 01 KARSHAM – Telugu of the following ingredient No. 4,5,6 and add 20 gms of GLYCERRHIZA GLABRA;

And

• “JEVANIYA GANA DRAVYAMULU” each 10 gms :

Mix it to above liquid and start cooking steady with steam stove carefully, when the whole volume comes to the volume of Ghee add 080 Gms Honey, and 080 Gms of powder of PIPER LONGUM add 0400 Gms of sugar and mix well till the entire liquid and after cooling preserve in suitable containers.

Dosage and Duration :

Take “01 Karsham” = 10 Gms of above medicine once daily or as directed by physician:  It is most effective remedy for controlling LOOSE MOTIONS WITH BLOOD.

“METHOD OF PREPARATION “PATYAADI VATUKAMULU”

• Take equal quantities  of items 1 to 8 and 23, 24, 25 make them in to fine pieces.
• Transfer the above in to a stainless steel wide mouthed open vessel and add upto (PRASTHAM = 640 Gms) (ML wise not mentioned) of PURE WATER.
• Steam boil stirring constantly so that a thick decoction is formed.
• Filter it via a new clean white cotton cloth in another  S.S. vessel.  To this decoction add fine powder.
• Of items nos 9 to 14 each one palamu = 40 gms.
• Add honey ¼th of the ORIGINAL VOLUME OF ABOVE DECOCTION ADD COW’S BUTTER MILK.
• 02 AADAKAMULU = 05 KGS AND 120 GMS.
• TO IT ADD – 08 PALAMULU = 320 GMS.
• Item No. 15, fine powder, un seeds item No. 16 SIXTY PIECES (NUMBERS) POWDER ADD COW GHEE.
• ONE KUDUVAM = 0160 GMS
• ONE KUDUVAM = 0160 GMS MUSTARD OIL STIRWELL THEN ADD.
• Fine powder of ZINGIBER OFFICIANALE one Kuduvam = 0160gms
• To the above preparation add one palamu each = 040 gms of powders of roots of ANDROPOGAN MURICATUS, MYRISTICA FRAGRANS M. M. OFFICIANALIS, PIPER NIGRUM AND COOK SLOWLY BYS -TEAM BY STIRRING constantly so that the contents are not over heated.
• Apply heat to such an extent a thick semisolid material remains in the vessel
• At this stage cool it and preserve well in GLASS JARS and keep them cool in dry place.

DETOXIFICATIN OF ARTICLES MENTIONED IN FORMULAE TO CONTROL DIARRHOEAS

As described in Ancient Ayurvedic Book BASAVA RAJEEYAM.

1.       POTHOS OFFICIANALIS (212)

          This material is to be SOAKED FULLY FOR 24 HRS in “KALI” mean RICE CUNGEE PRESERVED IN A VESSEL FOR 03 DAYS by adding beaten rice, It will have acidic taste and smell (from page 217 VASTUGUNA PRAKASHIKA – ARYURVEDIC BOOK).  Then it is to be taken and dried fully under SUN

2.       FERULA ASSAFOETEDA (67)

          Take pure juice of betel leaves and immerse above aritlce in that juice for 24 hrs.  Separate dry under,sun.

3.       CUMIUM CYMINUM CUMMIN SEEDS (306) : 

          Immerse this in “KALI” for 24 hrs.  Dry under sun, use.

4.       DATURA (ROOTS) METEL BLACK FLOWERS (89) VARIETY ONLY USE :

          Take pure juice of LIME JUICE (RIPE FRUITS ONLY) and immerse this product for 03 days. Separate dry under sun, use.

NOTES :

• Never use undetoxified ingredients.
• Follow strict hyegcine in handling materials.
• Make sure rodents, ants, insects donot spoil the procedure.
• These immersions to be done well ventilated rodent) protected room.
• Dry then under SUN LIGHT ONLY No other method to dry up in not useful.
• For lime juice never add water.
• For betel leaves little water can be added to get juice
